Open Doors works through the Supports for Community Living Medicaid Waiver (SCL) to serve people with intellectual and/or developmental disabilities. The SCL Waiver was designed as an alternative to institutional placement to keep people in their community across Kentucky. At Open Doors, we focus on Residential Level II- one service offered through SCL. Also known as Family Home Provider (FHP) or Adult Foster Care (AFC), we recruit, train and retain the most caring and qualified candidates to provide care and facilitate independence to the people we serve. Our ability to focus on what we believe is most important-home, offers the people we serve a well-honed level of expertise from our team of administrators, nurses and staff. Open Doors is certified to operate in all 120 Kentucky counties, and we have active providers in Louisville and Lexington as well as the Western and Eastern parts of the state. All of our caring and well-trained providers are eager to meet and service SCL participants across the state.

Using a person-centered team approach, Open Doors works to offer SCL participants choices of FHP/AFC that meet their social, spiritual and functional needs. Our process allows us to get to know an SCL participant to find the best fit- the best home for them, while ensuring Freedom of Choice. This holistic approach allows long-term relationships to form and grow over time resulting, not in a “placement” but in a home with the best supports and services.

Our Family Home Providers/Adult Foster Care Providers are carefully selected and trained to the highest standard. Our providers are a diverse group of caring professionals who work tirelessly to ensure those they serve have access to the highest quality of life in a community setting while attending to health, safety and welfare coupled with a commitment to satisfaction with services.

At Open Doors we strongly believe that intellectual and developmental disabilities shouldn’t hold anyone back from having a fulfilling and rewarding life, and the best way to achieve that aim is to encourage independence and community involvement through a shared living arrangement with a Family Home Provider.
